Alumni: How to Request a Replacement Diploma
To request a replacement high school diploma, please follow the steps below. Note: Both the request form and
payment must be completed before your request can be processed.
Step 1: Submit the Official Request
Complete the form, ensuring you select the option for Diploma (Mailed).
- Important: Please use your legal maiden name as it appeared at the time of your graduation.
Step 2: Complete Your Payment
Once the form is submitted, you must provide payment via the Herff Jones website ($50.00 payment)

Step 3: Verification & Shipping
- Payment Requirement: Processing will not begin until Herff Jones notifies the school that your payment has been received.
- Confirmation: Once payment is verified, the School Counseling office will confirm your graduation status and authorize the printing and mailing of your diploma.
